Can't speak for RCI or Azamara, but Celebrity has, in fact, begun imposing a 3% service fee for $$ taken from the casino, even if taken to gamble. A sign to that effect was posted at the casino cage...

At the end of a cruise is unused onboard credit charged back to the credit card used for your onboard account as a credit, or is OBC a matter of "use it or lose it?"
